The Canvas Live Shell Request Form was created to notify the Distance Education Department that a campus department would like a Canvas Live Shell. Please follow the instructions on the form while filling it out.
The Deans and Chairs Schedule Update Form will need to be filled out given the following circumstances:
- when online or hybrid courses are added after the finalization of the schedule
- when online or hybrid courses are listed as staff and then assigned late
- when there is a name change to an online or hybrid course
The Faculty Evaluation Request Form was created to notify the Distance Education Department that access to a faculty course shell is being requested for the sole purpose of fulfilling a faculty evaluation. This form should only be filled out by a Dean or Division Chair.
