Below are form links for various request that can be made to the Distance Education Department. In order to fill out the forms you will need to log into your "MyCompton" account.

Canvas Development Shell

Use this form to request a Canvas Shell for course development purposes

Faculty Sandbox Request Form

 

 

Course Merge Request

For faculty that are teaching multiple sections of the same course can request that they be combined into a single Canvas Shell.

Additional Users

Faculty interested in adding additional users to their courses such as observers, tutors, or teaching assistants should fill out this form.

Note: the Distance Education Department does not add faculty to courses they are not teaching.

Faculty Evaluation Canvas Access Request

This form is for Deans, Department Chairs, and Instructional Coordinators requesting access to a Canvas Course or schedule a student survey for the purpose of a faculty evaluation.

This form should be used to make updates to courses such as instructor change, course addition or cancellation, or course extension.
